About the Passport

A Practical Approach to Healthcare Continuity

Health information is often spread across physician offices, hospitals, pharmacies, patient portals, and paper records. When someone travels, relocates, changes providers, or requires urgent care, essential information may not be readily available.

The Healthcare Continuity Passport™ provides one organized place to maintain important health information, including medications, medical conditions, allergies, care contacts, recent healthcare encounters, emergency information, and supporting documents.

The Passport is inspired by the International Patient Summary and is designed to support—not replace—official clinical records or professional medical advice.

Customers complete and maintain the fillable digital Passport on their own device. They may print individual pages and insert them into the binder. When information changes, they update the relevant digital page, print the revised page, and replace the previous version. The customer stores the digital master copy in a personal cloud account or storage location of their choosing.

Full Paper Passport

For customers who prefer handwritten records

Customers complete and update the Passport manually. Additional pages may be requested when records expand or information changes. Customers are encouraged to periodically scan the completed Passport and save a PDF copy to personal cloud storage or another secure location of their choosing.

For customers who prefer an electronic record

Customers maintain the Passport electronically and may access it from a computer, tablet, or mobile device, depending on their chosen storage platform. SMG Global Health does not store the customer's completed Passport or personal health information.

The Healthcare Continuity Passport™ is based on a patient-controlled model.

SMG Global Health provides the structure, tools, forms, and educational guidance. The customer remains responsible for completing, reviewing, storing, updating, and sharing the information.

SMG Global Health does not retain completed Passport records or act as the custodian of the customer's personal health information.

You choose where your Passport is stored.

You decide who may access it.

You determine when information is updated.

You remain in control of how it is shared.

Optional Feature

Optional QR-Enabled Access

Customers may create a QR code that links to a digital copy of their Passport or to selected emergency information stored in a personal cloud account or secure document platform.

The QR code does not contain the health record itself. It directs an authorized user to the location selected by the customer.

Customers are responsible for the privacy settings, permissions, password controls, and continued availability of their chosen storage platform.

SMG Global Health may provide QR setup guidance or optional QR support services but does not store the underlying health information.

Need Help Completing Your Passport?

Some customers may prefer assistance organizing or entering their information. Optional Passport completion support may be available through trained support personnel. Depending on the service selected, assistance may include document organization, form completion support, medication-list preparation, page printing, and Passport assembly. Any clinical verification must be completed by an appropriately qualified healthcare professional.
